Plan of Care Details: Diagnosis, injury history, allergies, chronic medical conditions, dietary needs, choking history, indications of sickness, medications, and medication schedules.
Personal Care Preferences: Likes, dislikes, basic and negotiable routines, communication preferences, and behavioral intervention strategies.
Safety & Compliance: Human rights restrictions in place for the protection of the participant or others, and specific goals of services.
Financial & Administrative Health Data: Medicaid ID numbers, diagnosis codes, budget authorizations, and authorization reference numbers.
Ongoing Service Records: We collect data during service delivery, including shift location data, staff notes, goal progress reports, medication support reports, incident/behavioral reports, medical/dental visit records, and grievance reports.

Usage Data is collected automatically when using the Service.

Usage Data may include information such as Your Device's Internet Protocol address (e.g. IP address), browser type, browser version, the pages of our Service that You visit, the time and date of Your visit, the time spent on those pages, unique device identifiers and other diagnostic data.

When You access the Service by or through a mobile device, We may collect certain information automatically, including, but not limited to, the type of mobile device You use, Your mobile device unique ID, the IP address of Your mobile device, Your mobile operating system, the type of mobile Internet browser You use, unique device identifiers and other diagnostic data.

Shift Location Data & Electronic Visit Verification (EVV)

To comply with federal and state regulations (including the 21st Century Cures Act), Simpl Care collects and processes geographic location data from staff devices at the beginning and end of each service shift.

Service Verification: To confirm that scheduled services were provided at the authorized location.
Regulatory Compliance: To meet Electronic Visit Verification (EVV) requirements mandated by the Utah Department of Health and Human Services (DHHS) and Medicaid.
Safety and Accountability: To ensure the safety of both the participant and the care provider during active service hours.

How the Data is Handled: Shift location data is strictly used for administrative and compliance purposes. It is stored securely as part of the participant’s service record and is only disclosed to state auditors, Medicaid payers, or as required for legal and billing verification.

Tracking Technologies and Cookies

We use Cookies and similar tracking technologies to track the activity on Our Service and store certain information. Tracking technologies used are beacons, tags, and scripts to collect and track information and to improve and analyze Our Service. The technologies We use may include:
